Open Forms under Between sessions. The screen includes Send a form, My forms and Received forms. Share Intake and Onboarding or prepare your own questions.
In Send a form, under Admission or Onboarding, click Duplicate and edit. The editor opens with all its questions, named “Admission (my version)” or “Onboarding (my version)”.
Change, remove or add questions and click Save: the copy is stored in My forms and you send it like any custom form. The platform original does not change. The name, email and phone questions keep their identifier, so the response still creates or finds the client’s record.
Duplicating creates a custom form, and custom forms are part of White Label. When you click the button you see the card to activate it.
Under My forms, click Create form. Enter a name and optional description; the person reads that description when completing the form.
Enter a name and save again. The warning does not confirm the form was created.
Enter the text and choose its response type. Mark Required if it must be answered. Questions without text are discarded on saving; at least one written question must remain.
Write at least one question. The form description does not replace its questions.
For select, multiple select or wheel, enter comma-separated options. In a wheel, each option is an area with a scale. Review options before saving.
The warning identifies the question requiring at least one option. Enter options or choose a type that does not require them.
Click + Add question, use arrows to reorder and × to remove a question. The editor supports up to 40 questions. Review text, type and requirement before saving.
If you created custom fields in Clients, choose Also save in the client file field when needed. Responses only keeps the answer without that additional mapping. Saving the form does not fill the client field: a received, associated response is needed.
Click Save and check the message and My forms entry. Edit lets you review saved questions; changes are confirmed with Save. If responses exist, review before changing a question’s meaning.
In My forms, each form has Duplicate. It opens a copy whose name ends in “(copy)”, so you can build a variant without touching the form you already sent.
Client, link and email.
Click Copy link or Send by email on the form. Search and choose the client under For which client? The response will link to that record; an active toolbar client does not replace this choice.
You can click Send unassigned. Later review the received response and assign it to an existing record or create the client. Do not assume an unassigned link is already associated.
After choosing a client or continuing unassigned, check the copied-link message. Paste the URL into your communication channel. Copying does not send email or confirm receipt; it is also separate from a reusable open tool link.
Copy link for Intake or Onboarding creates an unassigned link. Send by email opens the client selector. From a client record, Send form lets you choose a form while keeping that client association.
Review the suggested email; you can correct it. Confirm when correct and wait for the result. Creating or copying a link does not request email delivery.
The dialog asks for an address without prefilling it. Enter a valid email or cancel and use Copy link. Associating a response does not require email in the client record.
Correct the address if rejected. If delivery fails, review the error: a link may have been created even though no email was sent. Do not interpret failure as a client response.
Assignment and archiving.
Under Received forms, click Refresh and open a response. Review form, date, details and assignment. Linked responses can also be viewed in the client record; sending creates no response until the person completes the form.
In an Unassigned response, select the correct record and click Assign. Check message and status. If the person is already a client, this avoids creating another record.
Select a client before clicking Assign. Opening a response does not link it automatically.
Click Create client in an unassigned response if no record exists. The process uses name and contact from the answers; if the name is missing, it asks for it. Then check the response is assigned.
Click Archive under My forms and read the confirmation. Sent links stop working and received responses are retained. Cancel if still awaiting responses or only editing questions.
Saving prepares the form; copying creates a link; sending by email requests delivery. None confirms that the person responded.
